What to bring to your first appointment
When you come to your first appointment, you’ll need to provide the following documentation:
- Photo ID of parent or legal guardian
- Your child’s insurance card, if he or she has one
- Your child’s immunization records if he or she has them
There’s also some paper work for new patients that you’ll need to complete. Please print and fill out the new patient forms and bring them to your first appointment.

Need to cancel? It is easy.
It is very easy to cancel an appointment. Just respond “No” to any text we send you to confirm your visit. Or, call us at 713-695-4013.
When you know you will not be able to keep a scheduled visit, it is vital to let us know so that the slot can be made available to another patient who needs it.

Tuesday: 8 a.m. - 7 p.m.
Wednesday - 8 a.m. - 7 p.m.
Thursday - 8 a.m. - 7 p.m.
Friday - 8 a.m. - 6 p.m.
Saturday & Sunday: closed
You can visit doctors and counselors from a smartphone, tablet, or computer. Call for a telemedicine or teletherapy appointment. Medicaid and most other insurance cover telehealth. If you don't have insurance, we can help!
