
What to bring to your child’s first appointment
When you come to your first appointment, you’ll need to provide the following documentation :
- Photo ID of parent or legal guardian
- Your child’s insurance card, if he or she has one
- Your child’s immunization records if he or she has them
Also, there’s some paper work for new patients that you’ll need to complete when you arrive. You can print and fill out the new patient forms ahead of time, if you wish. Click on your prefered language below:
Need to cancel? It is easy.
It is very easy to cancel an appointment. Just respond “No” to any text we send you to confirm your visit. Or, call us at 713-695-4013.
When you know you will not be able to keep a scheduled visit, it is vital to let us know so that the slot can be made available to another patient who needs it.
